Intensive Case Management is a community-based level of care aiming to provide long-term care for severely mentally ill people who do not require immediate admission. Our case managers are social workers, peers, and community support workers have experience working with people experiencing severe mental illness.
We employ strategies that include evidence based practices such as trauma informed care coordination and housing first.
